S & H health care services is licensed by the Minnesota Department of Health as a Assisted Living Care provider. Our Housing with Services(HWS) site, located in beautiful Golden Valley, is registered with Minnesota Department of Health. Our goal is to provide quality care to our clients in a home-like setting, with the goal of having them be as independent as possible.
Owner/Manager:
You will find that this owner and manager is involved in the day to day care of the clients. With one point person to address concerns, clients find things simplified in getting their needs met.
Nurses:
We have an experienced RN who makes home visits and is on call 24/7 days a week.
Caregivers:
Our staff are called personal care attendants (PCA’s) or unlicensed personnel (ULP). They are onsite 24 hours a day, 7 days a week to assist you tasks and ADL’s you need help with. Staff are trained and competency tested before providing services. Staff are dedicated, caring and attentive.
